East Toronto Health Partners' pre-registration portal for priority population vaccinations is now open. 

Priority population for the COVID-19 vaccine includes:
  • Seniors aged 80+
  • Those living in homecare
  • Indigenous adults
  • Community Healthcare Workers who qualify as "very high" and "high priority" as per the definition from the Ministry of Health (this includes those who are client-facing staff such as dentist offices, community clinics, pharmacies)

If you are 80 years or older, get homecare services and/or are an Indigenous Adult (over 16 years old), you are eligible for the COVID-19 vaccine.

If you live or work in East Toronto and you would like to receive your COVID-19 vaccine through East Toronto Health Partners (ETHP), please pre-register for your vaccine by completing this form.

What is the East Toronto Health Partners?
East Toronto Health Partners are responsible for providing care to the 300,000 individuals who live in East Toronto, as well as the many other people who choose to receive healthcare here. For more information on East Toronto Health Partners and our geographic boundaries, please visit www.ethp.ca

Pre-Registering means you will be contacted when you are eligible to get the vaccine. You will be able to book an actual appointment at that time. You will be contacted and given information on how to book an appointment directly with East Toronto Health Partners, or through other partners, as more options become available. The Ontario Ministry of Health decides who is eligible to get the vaccine.

  • Mental Health and Addictions Services
  • Campus Health
  • Community Diagnostic Imaging

If you are a patient/client facing Community Healthcare Worker and meet the Ministry of Health definition for Very High or High Priority, you are eligible to receive the COVID-19 vaccine. Please note that you can still complete this pre-registration form if you are a non-patient/client facing Community Healthcare Worker. You will be contacted for a vaccine appointment when you become eligible.
